Do All Teeth That Need a Root Canal Feel the Same?

No not all teeth that need root canal treatment present the same. Here at Pine Ridge Dental in Zimmerman, MN we know some are loud and painful and some are quiet and show up slowly.

At Pine Ridge Dental in Zimmerman, MN, we often hear patients say, “I know I need a root canal because this tooth hurts.” The truth is that not all teeth that need a root canal treatment (RCT) present the same way. Tooth pain can come from many different causes, which is why accurate diagnosis is critical. Sometimes a tooth can be abscessed and have no pain, but show bone loss on an xray.

Why Proper Diagnosis Matters

Treating tooth pain isn’t about guessing – it’s about understanding what’s happening inside the tooth and surrounding structures. Some teeth need a root canal, others do not, and in some cases, a simple bite adjustment can resolve the problem.

Tests We Use to Determine What’s Needed

At Pine Ridge Dental, we use a combination of clinical tests and imaging to determine the cause of pain:

  • Cold Test – Helps determine if the pulp (nerve) is healthy, inflamed, or non-responsive
  • Percussion Test – Tapping on the tooth checks for inflammation around the root.  Helps to determine pressure sensitivity
  • Palpation Test – Pressing on surrounding tissues helps identify infection or abscess
  • Digital X-rays & CBCT Scans – Allow us to see bone, roots, previous root canals, fractures, and infections not visible otherwise
  • Mobility Check – Looseness can indicate bone loss or trauma
  • Periodontal Pocket Evaluation – A localized deep pocket may point to a draining infection in the pocket space
  • Draining Fistula (Pimple on Gums) – Often a sign of chronic infection trying to drain through the tissue
  • Bite Evaluation – Trauma from an uneven bite pressure or parafunction can cause pain because of long term trauma on a tooth.
  • Crack Evaluation – Cracks can expand and cause the need for a root canal

Symptoms Tell an Important Story

Patients explanations of symptoms can guide diagnosis as much as testing:

  • Throbbing or aching pain
  • Pain when biting or chewing
  • Cold sensitivity that lingers
  • Pain with heat
  • No response to temperature at all
  • Minimal to No relief with adequate ibuprofen, acetaminophen or asprin

What Could Be Causing the Pain?

Based on findings, the diagnosis may include:

  • Necrotic pulp (dead nerve requiring RCT)
  • Vital pulp with irreversible pulpitis (inflamed nerve that cannot heal)
  • Traumatic bite injury causing reversible pulpitis, where adjusting the bite can relieve symptoms
  • Cracked tooth

Experience Matters in Diagnosis

Root canals aren’t determined by a single test or symptom. At Pine Ridge Dental, we combine technology, experience, and careful evaluation to recommend the right treatment—no more, no less.
